I hereby give my consent myself and or my child to to attend and participate in the following activity: Global Youth Day 2019 March 16, 2019 I understand that it may be necessary; to be transported in church approved vehicles or in vehicles driven by other licensed drivers’ to and from the activity locations: Located throughout Miami-Dade and Broward Counties on Saturday, March 16, 2019 I hereby voluntarily waive any claim against the Mt Pisgah SDA Church church staff, the Mt Pisgah Church, or the Southeastern Conference of Seventh-day Adventists for any accidents which may arise in connection with the activities of the Global Youth Day 2019 now and at any future time. USE OF MEDIA Waiver/Release We will be taking photo(s)/ and making video recordings of Global Youth Day (“GYD”) activities, in part to help us with promotion for the event and other similar events.  These images may appear in our printed publications, on our church website, social media or all of the aforementioned.  By participating in GYD, you are accepting these terms and you give permission for you and/or your child’s image to be used as stated. Use of Media By your participation in GYD, you give unrestricted permission for your image and that of your child (ren) to be used in print, video, and digital media. You agree that these images may be used by Mt. Pisgah Communications and Media Ministries for a variety of purposes and that these images may be used without further notifying you.
